Dr. Carly can help you understand what you are observing, connect you with the right resources, and prepare you for your neuropsychological evaluation so you know exactly what questions to ask and feel confident getting the answers you need to move forward. You do not have to wait to get support.

Many families also leave with practical language for the conversation with the rest of the family, and a better sense of what questions to ask other providers going forward. Every appointment is tailored to where your family is in the process, so what you walk away with depends on your starting point.
Memory Bridge is a private-pay practice and does not bill insurance directly. All fees are due at the time of service. Upon request, Dr. Carly can provide a receipt that you may submit to your insurance carrier for potential out-of-network reimbursement.

Yes. Dr. Carly offers both telehealth and in-person appointments. Telehealth sessions are conducted through a secure, HIPAA-compliant video platform and are available to clients located anywhere in Minnesota.

The Initial Consultation is the right starting point for new families. Dr. Carly specializes exclusively in dementia education, consultation, and support. She offers dedicated time to help you understand the diagnosis, navigate what comes next, and build a plan that works for your family.
Yes. Memory Bridge accepts HSA and FSA cards as payment. A receipt will be provided for your records after each appointment.
We understand that life with dementia is unpredictable. If you need to cancel or reschedule, please notify us at least 48 hours before your appointment. Cancellations made within 48 hours of the scheduled appointment, or missed appointments, will be charged 50% of the session fee. Please reach out as soon as possible and we will do our best to find a time that works for you.
Not at this time. Dr. Carly works full-time as a neuropsychologist at a Twin Cities hospital, and Memory Bridge is focused on providing dementia education, consultation, and family support.
